Exhibitor ​Application

This application must be completed and returned to 4 Ever Events by October 1st, 2018 in order to be included in the booklet given to all attendees.  Payment must also be included for the early bird booth rental fee of $100.00 per 8’ x 8’ space. The cost is $150 after September 1st. Payments made by checks or credit cards will be accepted.  The space includes one 5’ round or one 6’ rectangular table and 2 chairs if requested below.  It also includes your business name, address and phone number to be included in an Exhibitor directory to be given to all attendees.  Display ads are also available for an additional fee.  Electricity is also available. 
    How many booths will you need? Who will be attending? Will you need a 5' rnd. or 6' rect. table? Will you need 1 or 2 chairs?
